Long Term Care

Through the Adult Services portion of the program, team members

  • Promote and support independent community living through care consultation and coordination of waiver programs.
  • Case managers work with home and community-based programs and help people access, coordinate, and monitor needed medical, social, educational, and other services as they relate to the person’s assessed needs and preferences.
  • Relocation Screenings for clients leaving Nursing Homes.

HOW ARE REFERRALS MADE?

Referrals may be made by the individual needing services, family members, doctors, homecare providers, hospital staff and concerned community members. Individuals interested in services may call Scott County Long Term Care Intake at 952-496-8556.

WHAT ARE THE FEES?

Fees are based according to what program individuals are enrolled and financial eligibility requirements.
