Application Process
IF YOU ARE INTERESTED IN APPLYING FOR CASE MANAGEMENT IN THE DEVELOPMENTAL DISABILITIES UNIT, YOU NEED TO CALL
952-445-7751 AND ASK FOR CENTRAL INTAKE*
For children under the age of three:
The DD unit has social workers who specialize in resources for very young children. Your initial request for case management services will be given to one of these specialized workers by our agency’s Central Intake Unit. Once you have a completed school assessment, the worker will meet with you to determine your child’s eligibility. If your child is found eligible for services for individuals with developmental disabilities, the worker will work closely with you and your IFSP (Individual Family Service Plan) service coordinator and team to identify and locate the supports you need to support your child at home.
For children over age three and adults:
The DD unit has an intake worker who will receive your request for services from our Central Intake Unit and assist you through the agency intake and eligibility process. During the intake process you will learn about services and resources that the applicant may be able to access. You will also be required to complete some county paperwork and provide documentation of the individual’s disability. This information will be used in determining eligibility for the applicant. If the applicant is eligible, he/she will be assigned an ongoing social worker in the developmental disabilities unit.

A signed Tennessen warning and HIPAA Privacy Notice
A signed Consent to Release Information

*Note referral must be made by applicant or their legal representative. Assistance in making the referral may be offered if needed.
