Head Start, WIC, school or daycare.
Be sure to bring any forms you need with you.
 

What is a Child and Teen Checkup?

A Child & Teen Checkup is a preventative "Well Child" exam at YOUR clinic, by YOUR physician. The Checkup includes:

  • A complete physical exam, including height and weight.
  • A review of your child's development.
  • Hearing and Vision Checks.
  • Information on good health and nutrition.
  • Lab tests, including lead tests for toddlers.
  • Immunizations.
  • Referrals to other health professionals when appropriate.

When should my child get a Child & Teen Checkup?

  • Between Birth and 1 month.
  • At 2, 4, 6, 9, 12, 15 and 18 months.
  • At 2, 3, 4, 5 and 6 years.
  • Every two years after age 6.

When should my child go to the Dentist?

  • Within six months after first tooth erupts, or no later than age one.
  • Dental visits should be done every six months, or more often if your dentist recommends.
