10 Day Execution Exemption Notice and Notice of Intent to Levy on Earnings

If you desire the Sheriff to levy on the judgment debtor’s wages, please mail or serve the judgment debtor a completed form 10 days (if served) or 13 days (if mailed) prior to bringing the writ to the Sheriff.

Writ of Execution Instructions

This is our in-house service information sheet for levying on a bank account or wages. The top portion identifies the judgment debtor – the person owing the debt. If you want to levy on a bank account, complete that portion, or if you wish to levy on the debtor’s wages, complete that portion. At the bottom of the form is information about the party owed the money, the judgment creditor or their attorney. This part is used to contact you if there is a question, and the address we will use for correspondence.

This is our in-house service information form. Please complete and submit this form with every process being brought to the Sheriff for service. The top portion is related to the party being served. The requested information in the middle section is not required, but is used to provide additional clues to help locate respondents who may be avoiding service, and ultimately, expedite your service with a minimal service fee.
